  • Your phone number and a good time to call
  • Prior therapy experience, if any
  • How you were referred to me
  • Preferred appointment times
This will help me determine whether I am a good match for you, or to whom I might refer you if I am not.
Fees
My fees vary depending on the type and length of session. Please contact me for more information. I am an out-of-network provider for many PPO insurance plans. Clients pay the full fee directly to me. I am happy to provide a monthly statement so that you may seek any reimbursement to which you may be entitled.
Hours
I see clients Mondays & Tuesdays, 11:30am-7:30pm, and Wednesdays & Thursdays, 11:30am-5:30pm.
